Medical Records

Our Medical Records Department is located in Suite 107 of the Medical Office Building, 2500 Bernville Road, Reading, PA. Copies of medical records can be requested from this location Monday through Friday from 7:30 a.m. to 5:00 p.m., or call 610-378-2380.

Important Information

• Processing time for medical records is approximately five business days.
• Photo ID is required at time of pick-up.
• Request forms must be signed by patients 18 years of age and older.

You may also request medical records by downloading the Authorization for Release, Use and Disclosure of Protected Health Information (Autorizacion Para La Divulgacion, El Uso y La Revelacion De Informacion Protegida Sobre La Salud) form and submitting it via:
