About Life Lion EMS Community Paramedicine
Community Paramedicine is currently focused on patients with heart failure and chronic obstructive pulmonary disease (COPD), as well as those with high-risk conditions.
Research has shown that patients who receive at least one Community Paramedicine home visit after discharge have a significantly lower risk of readmission to the hospital than those who do not.
The transition home after being discharged from the hospital can be a challenging time for patients and caregivers. From filling prescriptions to following medication instructions and scheduling doctor’s appointments, it can be a stressful adjustment.
We’re here to help connect you with providers, address your concerns and provide the support you need for your continued health.
Who We Are
The Life Lion EMS Community Paramedicine team is made up of specialized paramedics who provide non-emergency, follow-up care. We assist your care team in evaluating your medical progress and ensure that you have access to every resource you need.
What We Do
A Community Paramedicine home visit will be scheduled before your discharge whenever possible. If you are discharged to a rehabilitation facility, we will contact you to schedule an appointment after your therapy is complete.
Community Paramedicine services:
- Occur at your home
- Are a component of your hospital care
- Are available to you at no additional cost
- Can be provided on short notice at your care team’s request.
What to Expect During a Home Visit
Your first home visit will usually take place in the first week after you return home. During this visit, your Community Paramedic will:
- Review your discharge instructions and medications
- Clarify your medical care plan
- Assist you with health system navigation
- Give you information about your disease or condition
- Perform a thorough physical exam
- Identify and address any ongoing medical needs you may have.
To aid your recovery, we will schedule a follow-up visit at your convenience and update your care team on how you are doing. If you experience a significant health issue, your care team can request an urgent Community Paramedicine home visit, and we will provide rapid assistance.
